The physical examination will focus on signs of medical conditions causing incontinence, such as tumors that block the urinary tract, stool impaction, and poor reflexes or sensations, which may be evidence of a related neurological cause.

The doctors of the Department of Urology Heilbronn attach a special importance to the information and the conversation with the affected patients in addition to a differential diagnosis and an interdisciplinary network to place the incontinence center at the SLK Klinikum Heilbronn provides a multi-institutional approach.

The most common forms of incontinence include the stress incontinence and the reflex incontinence.

Diagnosis

Bladder pressure measurement (video urodynamics)

A first test includes the measurement of bladder capacity and residual urine for evidence of poorly functioning bladder muscles.

The treatment of urinary incontinence and a neurogenic bladder based on the exact diagnosis of the malfunction of the bladder and / or sphincter. Various techniques measure pressure in the bladder and the flow of urine.

With the video-urodynamics can store and discharge of the lower urinary tract disorders are assessed differentiated. In combination with a digital X-ray image, the necessary radiation exposure is kept to a minimum, an important point especially for the pedriatric urology. We take an digital x ray image during the filled and empty period.

A comprehensive video-urodynamic measurement device that allows you to gain detailed information on the bladder, pelvic floor and sphincter function.

Therapies

Depending on the severity of urinary incontinence, a conservative (non-surgical) therapy may be tried. These include inter alia consistent pelvic floor training, which strengthens muscles and ligaments of the attachment apparatus sustainable. Another form of non-surgical therapy is the drug therapy.

If conservative treatments are insufficient, the Urology Heilbronn offers minimally invasive and gentle techniques to improve the symptoms of the patient.

Stress Incontinence

Characteristic of stress incontinence is a loss of urine when coughing, laughing and lifting heavy loads. The manifestations in severe and advanced diseases are seen in a loss of urine in the walking and lying down. It is a disorder of the sphincter function, which can be congenital. The reasons for this are often age-and depending on the weight or based on a connective tissue that occur after pregnancy and complicated labor. This disease may increase resulting after the prostate surgery by male patients.

After other treatments have been tried without success, we suggest surgery to alleviate incontinence. We prefer the minimally invasive surgical options and we have high rates of success in the female stress incontinence.

The use of small artificial taps (TVT, TOT) are proven as the standard therapy. Like a 'net' around the urethra, the tapes are supporting the ligaments. In complicated cases, an artificial sphincter (ATOMS, ADVANCE) is used. If these interventions do not restore the continence, the Urology Heilbronn offers the method of bundle plastic. From this body's connective tissue will be formed an envelope, which surrounds the muscles and thus strengthens.

In the male stress incontinence occurs in complicated cases, the ATOMS system is used. Using a small perineal incision the device is place below the urethra to restore the position of the bladder neck and to support the external sphincter muscle.

Urinary incontinence

The inability to hold urine or lose we call it involuntary bladder weakness or incontinence. The capacity ranges from single drops to a permanent loss of urine. The patient has been lost the ability to keep even the smallest amounts of urine.

If the conservative methods are exhausted, we offers successful surgical treatments.

The Department of Urology SLK Kliniken Heilbronn use successfully urethral slings as a method in the case of moderate diseases. The urethra in the perineal area is slightly raised and compressed in this way. The most important thing is to find out the best treatment for the patient.

Following systems have proven in use: The adjustable loop systems Heilbronn Argus ®(Orotech) and Atom ® (AMI) and the non-adjustable to sling implant AdVance ® (AMS)

Bladder prolapse

The Urology Heilbronn perform an opertation to selected indications like the bladder prolapse, by modern surgical technique: the robot-assisted system DaVinci®.

The DaVinci® system will be used to minimally invasive treatments such as a bladder prolapse and correction are performed simultaneously raising the vagina in the treatment of incontinence.

Patients with vaginal, intestine or bladder prolapse, there is a possibility of a laparoscopic Sacropexy. Patients with previous surgery (uterus-surgery) benefit from this non-incisional technology.

Reflex Incontinence

In the case of a reflex incontinence, the bladder and the function of sphincter muscle are out of control due to a neurological disease or injury of the corresponding nerves.

The protection of the kidney is very important during the treatment of incontinence. The conservative procedures will be included catheter systems.

One of the established surgical treatments at the Urology Heilbronn relates the 'Sacred Neuromodulation', using of a bladder pacemaker to stimulation the sacral nerves.
